Checkpoint inhibitors reshaped gynecologic oncology, and mismatch repair status became one of the strongest predictors of response. Jemperli (dostarlimab) was developed around that biomarker.
By preventing PD-1 from restraining T cells, Jemperli allows the immune system to recognize and attack tumor cells. The intravenous antibody was first approved for adults with mismatch repair-deficient recurrent or advanced endometrial cancer that progressed on a platinum-containing regimen.
Jemperli still carries an ongoing accelerated approval for mismatch repair-deficient solid tumors and has since moved into earlier endometrial cancer treatment in combination with chemotherapy. As a biologic it faces no biosimilar competition, and its rectal cancer data have made it a closely watched asset.
